The policy number is located on your certificate in the middle of the page under the "POLICY NUMBER" column. Providing this greatly helps our service staff.

In order to provide you with a quote, we must first get some information about your event:

If your upcoming event has been cancelled, first of all - we are so sorry! We will need your request to cancel your insurance in writing prior to the first date on the policy. You can email firstname.lastname@example.org or complete this form.
Once we receive your request, we are required to notify each of your certificate holders that the policy has been cancelled and that coverage no longer exists under the policy.
